Canine form

In an effort to ensure your preferences are recorded prior to the appointment please answer the following questions below. We will not vaccinate your pet or perform services prior to speaking with you. Capturing this information ahead of time helps us prepare the room prior to your arrival and advise of possible scheduling adjustments if multiple vaccines are requested.

Do you wish to have your pet kept current with what are considered “core” vaccinations (Rabies and DA2PP)?
Would you like to have your pet vaccinated against Lyme Disease?
Would you like to have your pet vaccinated against Leptospirosis?
Would you like your pet vaccinated against Bordetella (“kennel cough vaccine”)?
Would you like your pet vaccinated against Canine Influenza?
Would you like us to keep your dog’s heartworm test status current (ie: perform a heartworm test if needed?)
Would you like us to provide you with Heartworm pills at the end of the appointment? (if so six months or one year)
Would you like us to provide you with a Flea and Tick preventative at the end of the appointment? (if so six month or one year)
Does your dog have a history of allergies to vaccines?
Anything else you would like us to know prior to the exam?
